Survey Format

The HPQ-Select self-report survey is web-based and takes approximately 10 minutes for an employee to complete. The employer does not have access to individual responses, which are confidential. A minimum of 500 completed employee surveys is required for a participating employer. For employer’s that can’t meet this criterion, the IBI Health and Productivity Snapshot should be the preferred report.  Smaller employers should contact IBI’s President, Tom Parry, at tparry@ibiweb.org if interested in participating.
